Licensed practical nurse onboarding questions before your first assignment

Clarify licensed practical nurse orientation, access, documentation, supervision, and readiness checks before taking on a new provider assignment.

Separate offer conditions from work preparation

Gather education details, employment dates, and the license information the employer requests. If a posting uses both LPN and LVN, confirm which work location the application covers and where the employer checks eligibility.

Ask the employer which steps are offer conditions, which are orientation tasks, and who confirms each is complete. Keep the contact, requested document, submission channel, and status together. An uploaded document is not the same as completed verification, and an offer does not establish access to the systems or equipment needed for the work.

Ask for the role-specific orientation plan

Request the orientation schedule, assigned supervisor, documentation training, and process for confirming competency before new responsibilities. Keep credential-submission instructions separate from any public candidate profile.

Record the assigned supervisor or trainer, the work areas included, and the process for asking questions. Ask how orientation changes for someone moving from a different setting. If training is described verbally, request the relevant written instructions so your first assignment can be compared with the preparation the employer planned.

  • Who provides nursing supervision on each shift?
  • How are duties and competency checks explained during orientation?
  • Who confirms that I am ready for each new responsibility?

Understand the escalation and handoff process

Use an example of receiving an unclear assignment and clarifying it with the appropriate nurse. Explain how you kept the handoff accurate and recorded the next step rather than presenting a clinical decision outside your role.

Use a hypothetical version of that situation to ask the trainer how the local process works. Identify the responsible person, the communication channel, the record to update, and the handoff when that person is unavailable. A named escalation route is more actionable than assuming another department will notice an unfinished task.

Keep a preparation record you can act on

Confirm permitted duties through the relevant authority and employer process; do not infer identical responsibilities from LPN and LVN titles across locations.

Maintain a private list of required access, training completed, responsibilities reviewed, and open questions. Store only information you are permitted to keep and do not copy patient or internal operational records into personal notes. Before an unfamiliar assignment, check the preparation record with the designated supervisor. Confirm how questions and incomplete tasks should be handed over so your orientation record supports the actual work rather than becoming a checklist of attendance alone.

  • Do I have the approved access needed for this assignment?
  • Which duties have been reviewed and which still need preparation?
  • Who receives unresolved questions and the next handoff?
